The answer from them is :-

The NHS Delivery Forms are meant to be kept locally by both the supplier and the NHS Trust/hospital etc that the equipment is being delivered to for their personal records.

BiP only need to have copies of the IFA and IFB registration forms as well as a copy of the insurance certificate.

I think they are asking for manufacturers to return the forms to get on the MIA list.
