AHA american healt asocaiton says that nearly monophasic 360 joule = Biphasic 200 joule ;
I m little confuse that some Biphasic defibrillators have 360 joule value;
who ill help me???

Hi this is why, if you have only monophasic units the high voltage could be between 1600V until 2300V (35Amps and 50Amps). This can injury the heart by metabolism.
In addition, biphasic waveforms are associated with less injurious effects on hemodynamic performance and myocardial metabolism.
In the worst case if you use a monophasic unit you can provocate a fibrillation signal.The monophasic mode means the current runs only in one direction. In biphasic mode it would be the power distributed in to directions plus and minus.
I use defibs since 1989 as paramedic and no as service engineer.

Just because 360 J is available, that doesn't mean you have to crank it up to the max every time. Even paramedics are supposed to think, you know. That's why so many defibs have 1-2-3 in big numbers on them, I guess. think

Hundreds of thousands of lives have been saved by traditional (monophasic) defibrillators!

You are right, if there are no new technologies & models who will buy them. First they told us to do MRI, CT & XRAY to diagnose the diseases, now they are telling us that it is causing cancer & lot of radiation related diseases.
So every time they invent or manufacture something, there is nothing best than that invention, after some time when people become use to that product, suddenly new invention comes negating old technologies & products. This is marketting..
In our local language there is a proverb that SALESMAN KNOWS HOW TO SELL A COMB TO A BALD PERSON.. Got the idea.. :-)

You are right as we have a biphasic with 360.

What we have are european protocols which give us the levels to be administered with the pauses between for heart massage.

Someone has to make the decision and put it down in writing and if you have a mixture then they all have to be clearly marked as to which protocol to administer.

I have found that the real reason for the promotion of biphasic defibs is so certain "Resus Officers" can roam from hospital to hospital, convincing (frightening) the Board that all the old befibs need to be chucked out, and then taking a nice "little something" (from the gouging suppliers of the new equipment) before moving on.
