We use 2 ways of assessing risk.
One for maintenance, and the other for training.
Clinical Risk is affected by both breakdown and missuse but they are 2 different issues.
We went through our list of equipment and went through it from a maintenance point of view and categorised everything into high medium or low.
High risk devices MUST be maintained. Medium risk - preferably maintained, Low risk - breakdown only.
On Training the users must carry out a risk exercise on their equipment, but were 'assisted' in their decisions by us.
'Trained' Users must be COMPETENT to use well MAINTAINED (high risk) equipment.
