We have quite a lot of equipment out on loan. Generally we work closely with the Community Nurses, Respiratory Nurses, etc who are encouraged to provide details of any faults. If it a appears to be a patient problem, we discuss it with appropriate person and sometimes speak to the patient themself, to guide them through the correct use. Those that stick the infamous "Faulty" stickers are assumed to be ex nurses!

Getting staff (let alone patients) to report faults has been a problem for as far as I can remember. Sometimes, getting the nurses to trust you takes a bit of effort. In some strange places where I have worked, the nurses used to hide broken kit in case they were blamed for breaking it, and having the cost deducted from their pay packets (and when you're only on a 1,000 Riyals a month, it takes a long, long time to pay for a CTG transducer, believe me)! smile
