Yes Brian, I spent a few minutes trying to find it the other day and gave up. I think the supplies catalogue is a nightmare too. I was trying to find out some consumable pricing in the on-line catalogue. Again i gave up and reverted to the paper copy.

Hi guys Here is your link for the current PPQ form and it does work, I use it every week at least. For those who are unaware, question 14 of the current PPQ begins with 'Will' and not 'Is' as in the old version. Remembering of course that both are entitled June 2003. Again on a weekly basis I have to throw out the incorrect form from suppliers. Huw can you make the complete string part of the link as it is only half appearing as a clickable link, thanks. Hi Jim, I'm afraid it's not (easily) possible to fix the link due to those square brackets.
They break the code. I've never seen another site use square brackets in a URL.

URL Encoding '[ = %5B' and '] = %5D' I have never understood the point of this?

It's all to do with w3c standards. e.g. You can't validate a page with a non-encoded ampersand - which are used in so many dynamic applications (because it's a reserved character). You have to use But, back to PPQs...

&&& ... good coders always avoid the use of sloppy ampersands ( eg, in database fields etc.), as it is often reserved as an indicator for "macro substitution", and can have other meanings in various (programming) languages. The % character is also reserved. As are squared brackets [] and a few others. Their use in URI's should be discouraged! All is revealed here. But, back to PPQs... (why?) 
If you don't inspect ... don't expect.

Whilst we're on, anyone come across suppliers who have completed indemnity forms and amended the liability insurance cover required ......... downwards??
Nice try, how would they like us to send their price lists back with a 40% reduction!!
Needless to say we will not be doing business with them.
