The on line reporting system works very well indeed! We also take all phone calls 24/7 and they are usually investigated immediately (depending on risk)! It is our hospital policy that the end user must raise an on line request regardless of the call! I appreciate what Geoff says about the build up of "false events" but in our opinion a call is a call! Someone still has to attend! Everything is logged! This has also led to us to readily detect where further equipment training may seriously be needed
