Thanks John

I do notice that quite a few of these have passed their expiry. Does that make them invalid?

No doubt the person designated to update these has been subject to review and has been axed as part of the cuts - sorry efficiency savings think

Looks like the DOH forgot to add the NHS Delivery Note to the docs available from their site. It was usually simpler to get the suppliers rep to sign this if their company was on the MIA register. Have emailed the DOH contact Dominique Mia to query.

Rob,

Do we really need the NHS Delivery Note. It has been a bit of a pain getting reps to come back in to sign the thing off. After all the Indemnity agreement should cover any problems whilst the equipment is on site. I'm not sure how other depts. record loan equipment, but as well as creating a job with the details of the loan, we also put a label on the kit with the "expiry" date on it. The users should take responsibility for returning the kit to the rep.

Hi Bill

The version of the delivery note we used to use looked like the example below and didn't have a section for the rep to complete after the end of the loan period. The rep would just complete the details and sign the delivery note at the start of the loan at either From A or Form B section. We did the same as you though regarding logging the device, creating a job number, labelling, return, etc. And it was contained on just one side of A4 thus saving the odd rainforest!

Hi Rob,
We were pressganged into using the complete form. The reason being that we had to get the rep to sign the last section to say that the equipment had left the trust. I think we were taking the role of equipment mangement too far. I'm sure others will disagree? I suppose we need to decide how much we interfere with the running of the trust regarding medical equipment.
