How this happened, I will never know. End user said there was smoke coming off the vital signs monitor, and it isn't charging.... People.....

That would have been an interesting one to take a look at (inspect). Was the live line blown away by whatever caused that? What was the state of things at the mains plug end, I wonder? Hopefully the fuse blew. Did the monitor survive unscathed? To be honest that looks more like the sort of mishap that sometimes occurs "in the workshop" (eg:- an errant soldering iron).
If you don't inspect ... don't expect.
