That transformer you want, it sounds a bit of a rare beast. I had the same problem a while back when I wanted a transformer for one of my transmitter amplifiers that required about 1KV and required to supply about 1KW. I ended up using a 1:1 transformer and quadrupling the voltage, which gave approx 1150 – 1200 volts. There are plenty of 1:1 transformers around at reasonable prices. The other voltages would have to be derived from a separated transformer(s).
You didn't mention the power required? You can obtain some huge electrolytic caps that have high current ratings these days, again at reasonable cost. E.g. 450v 470uF 3.4A@100Hz £10.
