"Used to"? Don't tell me standards have started to slip out there, as well?  Anyway, have you never seen the TSG* in action? They still get exercised every now and then. Despite all the bleeding hearts, as long as "rent-a-mob" is a possibility (and, it always is), there always needs to be a "force of last resort" available somewhere. * The so-called "Territorial Support Group" of the Metropolitan Police "Service".

I might also add to my original post that, if the device in question will allow, download the event and error logs and include these in the report. This may well show if anything "odd" happened leading up to or at the time of the incident or, in some devices, what the settings being used were.

....more often than not It is quite rare that the equipment is the root cause of any "incident". It does happen (and for sure the staff involved are not going to hold their hands up to causing the problem) but as a biomed in these circumstances your role is only to appaise the functionality and safety of the equipment involved and report on that.... full stop. Was it operator error? not your call (hence my reference to NOT expressing opinion) that is up to others to decide.
