Any ideas of how i can write an writing an equipment service contract for a hospital, annual contract and for general medical equipment, ranging from simple ones like Aneroid Blood pressure machines to X-ray units, wish to service them on quarterly basis.

Before you write a contract you need to do a physical inventory to ascertain the status of the equipment, the inventory will assist you in determining the periodic maintenance schedules. Not all equipment needs quarterly maintenance.

A lot will depend upon whether you are drafting as a (prospective) Contractor, or as the Client (or Customer). 
If you don't inspect ... don't expect.
